Please provide all requested documents as soon as possible to expedite the timely closing of your loan.

1. Entity formation documents (ie. Articles of Agreement).
2. Certificate of good standing.
3. Current Organizational Agreement (who can borrow on behalf of the organization).
4. Guarantors Information (Address, phone #, SS#, DOB).
5. Property Access Instructions (who to contact for appraisal access)
6. Current appraisal or comparable properties (optional)
7. Escrow / Title Agent contact info (Company, Contact Name, Phone Number, Email Address)
8. Homeowners Insurance Agent (Name, Phone Number, Email Address)
9. Signed purchase agreements with all addendums
10. Completed Real Estate Transaction History form.
11. Completed Scope of Work Form
12. Major Rehab Tasks Review
13. Three months of most recent bank statements (all pages)
14. Voided check for ACH payment
15. Signed Completed Disclosure Statement (put property address at top of statement) I will send form.
